What a double chin is
A double chin is a fold or fullness beneath the chin that softens the angle between the jaw and the neck. It can affect people of any weight and any age. Some have had it since their teens because of genetics or face shape; others notice it developing later as skin loosens. It often shows most in photos taken from slightly below or when looking down at a phone, and it can make the lower face look wider than it is.
Why it happens and which layers are involved
There is rarely a single cause. In our 8-layer Skin Architecture model, three layers usually matter:
- Subcutaneous fat. Localised fat under the chin is often stubborn and does not always respond to diet or exercise. Genetics play a large part.
- Dermis and fascia. As collagen declines (around 1% a year from about 20) and supporting tissue slackens, skin under the chin loses firmness and begins to fold.
- Bone. A small or set-back chin gives less structural support, so even a little fullness looks more pronounced. Some people with a “double chin” actually have more of a recessed chin .
Posture also plays a role: a forward head position, common with screen use, makes the area look fuller. Knowing which factor dominates is what lets us choose an approach that actually helps.
How Obliv approaches a double chin
- Diagnosis first. A director examines the area from the side and below, gently assessing how much is fat and how much is loose skin, and looks at chin and jaw structure.
- Reducing fat. Onda uses microwave energy to selectively heat fat under the chin and below the cheeks, while sparing surrounding tissue. It suits stubborn, localised fullness.
- Tightening skin. When laxity contributes, HIFU such as Ultherapy PRIME or CoolSoniq lifts at depth, and RF such as Thermage tightens the dermis. Reducing fat without tightening can leave skin loose, so we often plan both.
- Restoring support. Where the chin is small or set back, chin filler can improve projection, making the jawline look clearer. Your doctor will explain whether this suits your proportions.
- Lifting. Thread lifts can add support along the jaw where sagging pulls tissue downwards.
What to expect
Energy treatments such as Onda, HIFU and RF have little downtime — perhaps mild redness, tenderness or swelling for a short period. Fat reduction and tightening develop gradually: changes are usually noticeable over 1–3 months, with the final result around 3–6 months. Filler shows immediately, with some swelling possible for a few days. Many people need more than one session, and results vary from person to person.
Keeping results
- A stable weight helps maintain the change.
- Raise screens to eye level to reduce constant chin-down posture.
- Plan tightening maintenance with your doctor, as collagen continues to decline with age.
Visiting from abroad
Onda and energy lifting usually involve minimal downtime, which makes them practical for a short stay. Send front and side photos on WhatsApp with your travel dates and we will suggest a plan.
Preparing for your consultation
A double chin can look very different depending on posture, lighting and camera angle, so a little preparation helps your doctor give you a more accurate plan:
- Bring a few photos. Photos from different years, or the angles where the fullness bothers you most, show whether the change is recent or long-standing.
- Note any weight changes. Recent gain or loss affects whether fat reduction, tightening or both make sense right now.
- Think about the whole lower face. Many people who mention a double chin are also bothered by a blurred jawline or sagging jowls. Mentioning everything lets the doctor plan one sequence instead of several separate treatments.
- Share your timeline. Because energy treatments develop over 1–3 months, tell us about any event or trip so sessions can be timed sensibly.
If your goal is a slimmer overall face shape rather than only the chin, our V-line face page explains how these approaches are combined.

Questions about double chin
I'm not overweight. Why do I have a double chin?
Genetics, chin shape, loose skin and posture can all create fullness under the chin regardless of weight. Your doctor will assess which factors apply to you.
How does Onda work?
Onda uses microwave energy to selectively heat fat in targeted areas such as under the chin and below the cheeks. Changes develop gradually over the following months, and results vary from person to person.
Will my skin be loose after fat reduction?
If there is already some laxity, reducing fat alone can leave skin less firm. That is why we often pair fat reduction with HIFU or RF tightening.
Is there downtime?
Energy treatments usually involve minimal downtime, perhaps mild redness, tenderness or swelling for a short time. Your doctor will explain what to expect for your plan.
