What sagging skin is
Sagging is the gradual downward shift of the face: the cheeks sit lower, the jawline softens, folds deepen and the face can look heavier or more tired than you feel. Some people notice it first around the jaw, others in the midface or along the neck. It often goes hand in hand with loss of elasticity , but the two are not quite the same — elasticity is about how skin springs back, while sagging is about structure moving downwards.
Why it happens and which layers are involved
At Obliv we describe ageing as working from the inside out. In our 8-layer Skin Architecture model:
- Bone gradually loses projection, so the framework that holds soft tissue shrinks.
- Deep fat and support tissue lose volume and ligaments loosen.
- Subcutaneous fat descends, creating heaviness lower down and hollowing higher up.
- Fascia (SMAS) , the supportive sheet that holds the face, slackens.
- Dermis thins as collagen falls by roughly 1% a year from about 20, faster after menopause and with sun damage.
Most people have changes in several layers at once, but one or two usually dominate. Treating the right depth is what makes a lift look natural.
How Obliv approaches sagging
- Diagnosis first. A director assesses your face from several angles, at rest and in movement, to judge which layers are driving the sagging. Mark-Vu imaging records skin quality alongside.
- Deep lifting (HIFU). Focused ultrasound delivers heat to precise depths (1.5, 3.0 and 4.5 mm). Ultherapy PRIME is our benchmark for deep sagging with multi-depth treatment; CoolSoniq reaches 4.5 mm with cooling for a gentler experience; OLIGIO HIFU reaches down to the SMAS.
- Dermal tightening (RF). Thermage heats evenly through the dermis for whole-face tightening; OLIGIO X targets the lower dermis for collagen remodelling.
- Mechanical lift. MINT Lift threads give an immediate lift, and JAMBER adds a scaffold that supports volume and ligament support.
- Volume and collagen. Where hollowing contributes, filler restores structure; collagen stimulators such as PCL or PLLA improve firmness gradually. Energy lifting combines well with collagen stimulators — see Energy Lifting + Collagen Stimulator .
What to expect
After HIFU or RF, many people feel a slight pulling sensation straight away. Change becomes noticeable over 1–3 months, with the final result at around 3–6 months as collagen rebuilds. HIFU results typically last around 12–18 months, RF around 6–12 months. Threads lift immediately and continue to encourage collagen as they dissolve. Discomfort varies by device and person; CoolSoniq tends to be more comfortable. Results vary from person to person, and your doctor will be honest about what non-surgical lifting can and cannot achieve.
Keeping results
- Protect collagen with daily SPF50+.
- Plan maintenance around how long each treatment lasts.
- Your Skin Library record lets us fine-tune depth and settings each time.
Visiting from abroad
Energy lifting has little downtime, so many visitors combine it with other treatments in one trip. Message us on WhatsApp with your dates and we will help you plan.
HIFU, RF or threads? How the options compare
Sagging can be treated at several depths, and the options are often combined rather than chosen one against another. As a general comparison:
- HIFU focuses ultrasound energy at specific depths, reaching as far as the SMAS. It suits deeper laxity, has little downtime and results typically last around 12–18 months.
- RF heats evenly through the dermis to tighten skin and stimulate collagen. It suits looser, thinner skin and typically lasts around 6–12 months.
- Threads lift tissue mechanically and give an immediate change, then encourage collagen as they dissolve. Some swelling or tenderness for a few days is possible.
- Collagen stimulators and filler rebuild support and volume where hollowing makes sagging look worse.
Many plans pair a deep treatment with a dermal one, as in our Ultherapy and Thermage combination . If you are new to lifting treatments, our non-surgical face lift page walks through how a plan is built over time.
At a glance
- Layers: Dermis, fat, fascia (SMAS), bone
- Main tools: Ultherapy PRIME, Thermage, threads
- Results visible: 1–3 months; final at 3–6 months
- HIFU lasts: Around 12–18 months
- RF lasts: Around 6–12 months

Questions about sagging skin
What is the difference between Ultherapy and Thermage?
Ultherapy PRIME uses focused ultrasound (HIFU) to heat precise points at depth, reaching the SMAS layer for deeper lifting. Thermage uses radiofrequency to heat evenly through the dermis for overall tightening. Many people benefit from both — see our Ultherapy + Thermage combination.
When will I see the lift?
You may feel a slight pulling straight after. Change is usually noticeable over 1–3 months, with the final result around 3–6 months. Results vary from person to person.
Does energy lifting hurt?
Sensation varies by device, area and person. CoolSoniq includes cooling and is generally more comfortable. Your doctor will discuss comfort options before treatment.
Is non-surgical lifting enough for me?
It can help many degrees of sagging, but it does not replicate surgery. Your doctor will give an honest assessment of what is realistic for your face at consultation.
