What a drooping midface is
The midface is the area between the lower eyelids and the corners of the mouth, including the cheeks. In youth, the fullest part of the cheek sits high, close to the cheekbone, and the face has a gentle heart shape. As the midface descends, that fullness moves down and inwards. The result is a flatter upper cheek, a more visible groove under the eye, deeper nasolabial folds and, eventually, heaviness at the jawline.
Many people notice that their face looks squarer or more tired, even though no single line has appeared. That is a typical sign that support in the midface has changed.
Why it happens: the layers involved
Ageing generally works from the inside out. Using our 8-layer Skin Architecture model, from epidermis to bone, the sequence often looks like this:
- Bone. The cheekbone and the bone around the eye socket slowly remodel, reducing the platform the soft tissue sits on.
- Deep fat. The deep fat pads of the cheek shrink, so there is less structure holding the upper cheek in place.
- Support tissue and fascia. Ligaments and the fascia layer (including the SMAS) loosen, letting the superficial fat slide down.
- Dermis. Collagen falls by roughly 1% a year from about 20, so the skin itself becomes less firm and cannot hold the new shape.
Because several layers are involved, treating only the surface rarely gives a satisfying lift.
How Obliv approaches a drooping midface
- Assessment by depth. A director examines how much of the change is lost volume and how much is laxity, and at which depth. This decides the order of treatment.
- Structural support first. Following a dual-plane approach, support is restored just above the bone, then in the fat layer. Cheek filler or CaHA filler can help lift the upper cheek from below; HA typically lasts 6–18 months, CaHA 12–18 months.
- Deep lifting with HIFU. Focused ultrasound reaches depths of 1.5, 3.0 and 4.5 mm. Ultherapy PRIME is the benchmark for deeper sagging; OLIGIO HIFU also targets the SMAS. Results build over 3–6 months and may last around 12–18 months.
- Firming the skin. Radiofrequency such as Thermage tightens the whole dermis, with results lasting around 6–12 months.
- Mechanical lift. Where an immediate change is wanted, MINT Lift barbed threads can reposition tissue and encourage collagen as they dissolve.
What to expect
Energy-based lifting may give a slight pulling sensation straight after, with noticeable change over 1–3 months and final results at 3–6 months. Filler and threads show change on the day, with possible swelling or bruising. On the day of injections or threads, avoid touching the area for 4–6 hours and skip saunas, alcohol and strenuous exercise. Comfort varies; CoolSoniq is a gentler HIFU option for those who are sensitive. Results vary from person to person.
Keeping results
- Protect collagen with daily SPF50+.
- Consider collagen stimulators, which combine well with energy lifting.
- Plan maintenance roughly in line with how long each treatment lasts.
Visiting from abroad
Midface lifting fits well into a short trip, because most energy treatments have little downtime. Send front and side photos plus your travel dates on WhatsApp and we will suggest a plan for your stay.
Why the order of treatment matters
A common mistake with midface sagging is to start at the surface or at the lowest point of the change. Filling a deep nasolabial fold on its own, for example, can add weight to an area that is already sliding down, and the result may look heavy rather than lifted. Tightening the skin alone may improve firmness but cannot replace support that has been lost near the bone.
That is why your doctor decides depth first and device second. A typical sequence might be:
- Restore structural support high on the cheek, close to the bone
- Lift the fascia and deeper tissue with HIFU or threads
- Firm the dermis with radiofrequency or collagen stimulation
- Refine any remaining lines once the overall shape has improved
Not everyone needs every step. Some people only need one or two, depending on what the assessment shows. If you are also concerned about the lower face, see our page on sagging jowls .

Questions about a drooping midface
Should I choose filler or lifting?
It depends on whether volume loss or laxity is the main cause. Many people benefit from both: support first, then lifting. The doctor will recommend the right order at consultation.
Is Ultherapy painful?
Sensation varies from person to person. If you are sensitive, CoolSoniq is a gentler HIFU option with cooling. The doctor will discuss comfort before treatment.
When will I see the lift?
A slight pulling sensation is common right after energy lifting, with noticeable change over 1–3 months and final results at 3–6 months. Filler and threads show change on the day.
Can it help my nasolabial folds?
Often, yes. Restoring midface support may soften the folds below, which is why we look at the cheek before treating the fold directly.
