What facial asymmetry is
Every face has two slightly different halves — one eyebrow sits a little higher, one cheek is a touch fuller, one side of the jaw is wider. Usually these differences go unnoticed. Facial asymmetry becomes a concern when the difference is clear enough that you see it in photos, in the mirror or when you smile. Common examples include one cheek that looks flatter, an uneven jawline, a brow that sits lower, or a smile that pulls more to one side.
Asymmetry can be present from birth, develop with habits such as chewing on one side or sleeping on one side, or appear with age as the face changes unevenly. Sudden new asymmetry, especially with weakness, numbness or pain, is a medical issue and should be checked by a doctor promptly rather than treated cosmetically.
Why it happens: which layers are involved
Our 8-layer Skin Architecture model, from epidermis to bone, helps pinpoint the source:
- Bone. Small differences in the cheekbone or jaw are common and set the basic frame. Ageing changes bone gradually, and not always equally on both sides.
- Fat and support tissue. Deep fat can shrink and descend more on one side, creating a flatter cheek or deeper fold.
- Muscle. Chewing more on one side can enlarge one masseter muscle, making the jaw look wider on that side. Stronger expression muscles on one side can raise or lower a brow or pull the mouth.
- Skin. Uneven sun exposure — for example, from driving — can make skin laxity and lines more pronounced on one side.
How Obliv approaches facial asymmetry
- Detailed assessment. A director studies your face at rest and in motion, and photos help compare both sides. Understanding whether the cause is bone, fat, muscle or skin is essential, because each needs a different approach.
- Balancing muscle. Where one muscle is stronger, Botox can relax it to even out a smile, brow or jaw. Jawline Botox is often used for a one-sided wide jaw. Botox starts working in 3–7 days, peaks around 2 weeks and lasts about 4–6 months.
- Restoring volume where it is missing. Using a dual-plane approach — support close to the bone first, then the fat layer — HA filler or cheek filler can be placed on one side or in different amounts on each side. HA filler typically lasts around 6–18 months.
- Lifting uneven laxity. If one side has descended more, energy-based lifting or MINT Lift threads may be used with adjusted settings for each side.
The aim is a more balanced face, not perfect symmetry, which would look unnatural.
What to expect
Filler and threads show changes on the day; Botox takes a week or two to settle, so we often review the result at around two weeks and fine-tune if needed. Mild swelling or bruising can occur. Avoid touching treated areas for 4–6 hours and skip saunas, alcohol and strenuous exercise on the day. Results vary from person to person.
Habits that help
- Try to chew on both sides and avoid habitually resting your chin on one hand.
- Alternate sleeping sides where possible.
- Use SPF50+ daily, including on the side exposed when driving.
Visiting from abroad
Because Botox reaches its peak at around two weeks, longer stays allow a review appointment; for shorter trips we will plan conservatively and advise on follow-up. Send front-facing photos at rest and smiling, plus your travel dates, on WhatsApp.
Setting realistic expectations
Asymmetry treatment works best in small, measured steps. Because the two sides of the face are being treated differently, your doctor will usually start conservatively and add more only if needed, rather than trying to match both sides in one session. This keeps results natural and avoids overcorrecting one side.
It also helps to know what injectables and energy treatments cannot change. Significant differences in the jaw or cheekbone that come from bone structure can often be softened visually, but not fully corrected without surgery. Your doctor will explain honestly which parts of the imbalance can be improved and by how much. For more on how the first appointment works, see what to expect at consultation .

Questions about facial asymmetry
Can non-surgical treatment correct asymmetry?
It can help improve many soft-tissue and muscle-related differences. Significant bone asymmetry may be beyond what injectables can balance, and the doctor will explain this honestly at consultation.
Will both sides be treated the same?
Usually not. The point is to treat each side according to what it needs, so doses and amounts are often different on the left and right.
My asymmetry appeared suddenly. Can you help?
Sudden changes, especially with weakness, numbness or pain, need prompt medical evaluation rather than cosmetic treatment. Please see a doctor first; we can discuss aesthetic options once the cause is clear.
Do I need a follow-up visit?
A review at around two weeks is helpful when Botox is used, as that is when it peaks. If you are only in Seoul briefly, we plan conservatively and can advise on follow-up from abroad.
