Hollow cheek treatment in Seoul

Hollows below the cheekbones can make the face look gaunt, tired or older than you feel. We rebuild support in the right layers so the cheeks look softly full again — not overdone.

What hollow cheeks are

Hollow cheeks are an inward dip in the area below the cheekbones and in front of the ears. A little definition here can look striking, but deeper hollowing tends to make the face look drawn, tired or unwell, and it can make the cheekbones and jaw look harsher than they really are. Hollow cheeks are common in slim people, after weight loss, in keen runners and cyclists, and as part of normal ageing. Many people also notice that makeup and lighting can no longer disguise the shadow.

Why they develop and which layers are involved

In our 8-layer Skin Architecture model, hollow cheeks are mainly a volume issue deeper in the face, rather than a skin problem:

  • Bone. The bones of the midface and upper jaw slowly lose projection with age, reducing the framework that supports the cheek.
  • Deep fat. Deep fat pads beneath the cheek shrink, removing cushioning close to the bone.
  • Support tissue. Ligaments loosen, so the remaining fat shifts downwards — creating hollowness above and heaviness lower down, such as jowls .
  • Subcutaneous fat. Weight loss or high activity levels reduce the more superficial fat too.
  • Dermis. With declining collagen (roughly 1% a year from about 20), skin drapes more closely over the hollow, making it more visible.

How Obliv approaches hollow cheeks

  • Diagnosis first. A director assesses your face from front, side and three-quarter angles, at rest and smiling, to understand which layers have lost volume and how the cheek relates to the temples, eyes and jaw.
  • Dual-plane volume. We restore structural support just above the bone first, then the fat layer, and consider muscle last. Cheek filler is placed with this order in mind so the result looks natural in movement.
  • Choosing the product. HA filler is soft and adjustable and typically lasts around 6–18 months. CaHA offers firmer support, lasts around 12–18 months and also stimulates collagen.
  • Gradual restoration. PLLA or PCL collagen stimulators can build volume slowly over months — a good option for people who prefer a gradual change.
  • Lift and volume together. JAMBER threads provide a scaffold that lifts and adds volume while encouraging collagen and ligament support.

Hollow cheeks often appear alongside sunken temples or flattened cheekbones ; treating neighbouring areas together keeps the face balanced.

What to expect

Filler shows immediately, with some swelling or bruising possible for a few days. Collagen stimulators start showing results around 1–3 months, peak at 3–6 months and can last around 12–24 months. On the day, avoid touching the area for 4–6 hours and skip sauna, alcohol and strenuous exercise. Results vary from person to person.

Keeping results

  • Plan top-ups around the lifespan of your chosen product.
  • Use daily SPF50+ to protect collagen.

Hollow cheeks after weight loss or intense training

A growing number of people notice hollow cheeks not because of age, but after losing a significant amount of weight or taking up endurance sport. The face often loses fat earlier and more visibly than the body, so even a healthy change can leave the cheeks looking sunken or tired.

A few points are worth knowing if this applies to you:

  • Timing. If you are still losing weight, your doctor may suggest waiting until it has stabilised, so the plan reflects your face as it will be.
  • Skin quality. After larger weight changes, the skin may also have lost some firmness. Combining volume with collagen support, rather than volume alone, often gives a more natural result.
  • Subtlety. The aim is to soften the shadow and restore a healthy look, not to change your face shape. Many people prefer a gradual approach with collagen stimulators for exactly this reason.
  • Activity. If you train hard, mention it at consultation. It can affect how long results last and how you plan maintenance.

Questions about hollow cheeks

  • Will cheek filler make my face look bigger?

The aim is to restore what has been lost, not to add bulk. By supporting the deeper layers first and using measured amounts, the face usually looks fresher rather than larger.

  • HA, CaHA or a collagen stimulator — which is right for me?

HA is soft and adjustable, CaHA gives firmer support and stimulates collagen, and PLLA or PCL build volume gradually. Your doctor will recommend based on your anatomy and how quickly you want to see change.

  • I'm slim and active. Is this common?

Yes. Low body fat and endurance exercise can make cheeks look hollow earlier. Treatment can restore softness without affecting your lifestyle.

  • How long does it last?

HA filler typically lasts around 6–18 months, CaHA around 12–18 months and collagen stimulators around 12–24 months. Results vary from person to person.
